We are looking for a dynamic and compassionate Case Management Coordinator to join our dedicated team. As a Case Management Coordinator, you will play a pivotal role in assessing, planning, and coordinating all aspects of case management activities to ensure the overall wellness of our members. Responsibilities
Conduct telephonic and/or face-to-face assessments with members to evaluate their medical needs and develop proactive care plans.
Manage a diverse caseload, including supportive and medically complex members, to enhance short and long-term outcomes.
Determine appropriate services and supports for members, such as prior authorizations, coordination with primary care providers and skilled providers, medication review, and access to community resources.
Facilitate optimal outcomes for members and identify opportunities for continuous quality improvement.
Educate and empower members to ensure compliance, satisfaction, and promote patient advocacy.
Collaborate with colleagues at all levels to optimize total costs and ensure policy adherence.
Requirements
Previous experience in case management is required; long-term care experience is preferred.
Proficiency in Microsoft Office, including Excel.
Bachelor’s degree in social work or related field is required; Master’s degree is preferred.
Work Environment
This position requires 50-75% travel for face-to-face visits with members in Miami-Dade County. However, it also offers the flexibility of a Monday-Friday schedule with standard business hours.
