About the position
We Care Home Health is seeking a compassionate and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide specialized care for an extended care patient with trach, vent, and G-tube needs. This role involves working night shifts in the patient’s home, ensuring their safety, comfort, and medical stability. Responsibilities
Delivering specialized care for a patient with trach, vent, and G-tube

Monitoring the patient’s respiratory and nutritional health during your shifts
,
Administering medications and tube feedings
,
Performing routine assessments, interventions, and adjustments as necessary
,
Communicating with the care team and maintaining accurate patient records
,
Developing strong, supportive relationships with the patient and their family
Requirements
Current Colorado LPN or RN license
,
Experience with trach, vent, and G-tube care preferred
,
Current BLS/CPR certification
,
Strong clinical and communication skills
,
Ability to work independently in a home care setting
,
A compassionate, patient-focused approach to nursing
