Job Overview
We are looking for a committed individual to join our Veterans Directed Home and Community Based Services Program (VD-HCBS) as a VA Case Manager. In this remote, part-time role, you will collaborate closely with the Program Manager to improve both fiscal and programmatic functions, ensuring that our veterans receive comprehensive and effective services.
Key Responsibilities
Deliver engaging training sessions for veterans and their representatives.
Conduct quarterly home visits for assigned veterans to assess and fulfill their needs.
Manage the intake process for new VD-HCBS referrals, including submission of enrollment documentation to appropriate financial management services and the Department of Veterans Affairs.
Maintain accurate records and correspondence related to VD-HCBS referrals, ensuring thorough documentation of veteran files.
Review and verify veterans timesheets, ensuring precise data entry within the electronic database.
Develop and evaluate budget plans for veterans to create accurate financial reports.
Assess veterans needs and ensure that provided services are effective and suitable.
Formulate improvement or corrective action plans as needed.
Promptly communicate with the Program Manager and financial services regarding any veterans institutional placement.
Work collaboratively with the Program Manager on new enrollments, changes in veteran status, and issue resolutions.
Address veteran-related issues and assist in the approval of necessary paperwork.
Coordinate with financial management services concerning enrollments and budget plan adjustments.
Prepare ad hoc reports as required.
Update and submit the VD-HCBS Monthly Update Spreadsheet by deadlines.
Conduct background checks for all employees chosen by the veteran for employment.
Maintain a summary page for each veteran, keeping it current as needed.
Submit a detailed spreadsheet of home visit and phone call notes by the specified deadline each month.
Complete mileage reports accurately and timely.
Attend meetings and conference calls with local and national VA representatives.
Uphold all security and confidentiality regulations.
Execute other duties as assigned by the Program Manager or other leadership.

Qualifications
An Associates or Bachelors degree is preferred, along with a minimum of 2 years of experience in case management. A combination of relevant education and experience will also be considered.
