Job Description
CVS Health is seeking a dedicated and compassionate Remote RN Health Coach to join our dynamic team. As a Remote RN Health Coach, you will play a crucial role in empowering patients to achieve their health goals by providing personalized coaching and support. You will work from the comfort of your home, using your expertise to make a positive impact on patient health outcomes.
As part of our team, you will be responsible for developing individualized care plans, educating patients on chronic disease management, and assisting them in making healthier lifestyle choices. This position requires excellent communication skills, a strong understanding of clinical guidelines, and the ability to build rapport with patients remotely.
Requirements
– Active RN license in the state of residence
– Minimum of 3 years of clinical experience, preferably in a coaching or case management setting
– Proficiency with telehealth platforms and electronic medical records (EMR)
– Strong understanding of chronic disease management and preventive care
– Exceptional communication and interpersonal skills
– Ability to work independently and manage time effectively
– Basic computer skills, including proficiency in Microsoft Office Suite
Responsibilities
– Develop and implement personalized care plans for patients
– Provide education and support on chronic disease management and preventive care
– Establish rapport and build trusting relationships with patients remotely
– Monitor patient progress and adjust care plans as necessary
– Collaborate with other healthcare professionals and providers to ensure comprehensive care
– Document patient interactions and maintain accurate records in the EMR
– Participate in ongoing training and professional development opportunities

Educational Qualifications
– Bachelor? Degree in Nursing (BSN) required; Master? Degree preferred
Experience
– Minimum of 3 years of clinical experience required
– Experience in health coaching, case management, or chronic disease management preferred
